Requirements:

An existing MPF/Citizen character
If Citizen, Loyalist Status.
If Citizen, delete your character or it will be deleted through database.
If MPF, Brainwashed. (01 and above).
If MPF, More than three achievements.
Go back to the SeC after being accepted, and tell him you're there for the instatement.
Ask for a Digit change and whitelist to CA.

After Being Whitelisted

Notes:

The City Administrator has next to no power over MPF Units. Ranks 01 and Above can override the orders of a CA.
If the application disappears from the main Application forums, and is not in the Denied sub-forum, then it has been accepted. However, it will be moved only 2 days after accepting it.
You need to apply multiple times for every CA character you want.
