Requirements:

An existing MPF character
Being in a Division, and at least being 01 Rank in that division.
Go back to the SeC or CA after being accepted, and tell him you're there for the instatement.
Ask for a Digit change and a 3-rank demote seeing as though the current char is being made into OTA.

If the application disappears from the main Application forums, and is not in the Denied sub-forum, then it has been accepted. However, it will be moved only 2 days after accepting it.
You need to apply multiple times for every OTA character you want.
